WAIVER, RELEASE OF LIABILITY & MEDIA CONSENT

1. PARTICIPANT INFORMATION

Before participating in the Elite National Camp, all participants must provide the required participant information through the registration form.

If the participant is under 18 years of age, the person completing the registration and accepting this waiver confirms that they are the participant's parent or legal guardian and have authority to accept these terms on the participant's behalf.

2. ASSUMPTION OF RISK

I acknowledge that participation in the Elite National Camp involves physical football activities and carries inherent risks.

These risks may include, but are not limited to, physical injury, illness, collisions, falls, contact with other participants, muscle strains, sprains, fractures, concussion, and other injuries or risks associated with football and sporting activities.

I understand that participation in the Elite National Camp is voluntary and that I am choosing to participate, or permitting the participant named in the registration to participate, with knowledge of these inherent risks.

I voluntarily assume all risks associated with participation in the Elite National Camp.

3. WAIVER AND RELEASE OF LIABILITY

To the extent permitted by law, I release and hold harmless Elite Pacific Sports, the Elite National Camp organisers, coaches, staff, volunteers, contractors, representatives, and associated parties from claims, liabilities, losses, damages, costs, or expenses arising from or connected with participation in the Elite National Camp, including injury, illness, or loss occurring during or in connection with the camp.

I understand that football and sporting activities involve inherent risks and that injuries or other incidents may occur despite reasonable precautions being taken by the organisers.

4. MEDICAL TREATMENT AND EMERGENCY ASSISTANCE

I understand that participation in sporting activities may involve the need for first aid or medical assistance.

In the event of an injury or medical emergency, I authorise the camp organisers and their representatives to arrange or obtain reasonable first aid, emergency assistance, or medical treatment for the participant where considered necessary.

I understand that I am responsible for providing accurate and complete information regarding any relevant medical conditions, allergies, medications, injuries, or other information that may be important to the participant's safety.

5. MEDIA, PHOTOGRAPHY AND CONTENT RELEASE

I acknowledge that photographs, videos, recordings, and other content may be taken during the Elite National Camp.

I grant Elite Pacific Sports and the Elite National Camp organisers permission to photograph, record, reproduce, edit, publish, distribute, and otherwise use images, video, audio, or other content featuring the participant for promotional, marketing, advertising, social media, website, educational, or other legitimate purposes connected with the organisation and promotion of the camp and its activities.

I understand that I will not receive payment or other compensation for the use of such content and, to the extent permitted by law, waive any right to inspect or approve the finished content before it is used.

6. PARTICIPANTS UNDER 18

If the participant is under 18 years of age, the person accepting this waiver confirms that they are the participant's parent or legal guardian and have the legal authority to provide consent and accept these terms on the participant's behalf.

By checking the acceptance box below, the parent or legal guardian agrees to all provisions of this waiver on behalf of the participant.

7. WEBSITE ACKNOWLEDGEMENT AND ACCEPTANCE

PLEASE READ THIS WAIVER CAREFULLY BEFORE ACCEPTING.

By checking the acceptance box on the Elite National Camp registration website, I confirm that:

• I have read and understood this Elite National Camp 2026 Waiver, Release of Liability & Media Consent.

• I understand that participation in football and sporting activities involves inherent risks, including the risk of injury or illness.

• I voluntarily accept and assume those risks.

• I agree to the waiver and release provisions contained in this document, to the extent permitted by law.

• I consent to the photography, video, recording, and other media provisions described above.

• I understand and agree that checking the acceptance box constitutes my acknowledgement and acceptance of this waiver and its terms.

• If I am accepting this waiver on behalf of a participant under 18, I confirm that I am the participant's parent or legal guardian and have authority to accept these terms on their behalf.

• I confirm that the information provided during registration is accurate and complete.

8. ELECTRONIC ACCEPTANCE

I acknowledge that my electronic acceptance of this waiver through the Elite National Camp registration website is intended to record my acknowledgement and agreement to the terms contained in this document.

No physical signature is required where the waiver is accepted electronically through the registration process.
